We collect information you voluntarily provide when you fill out a contact form, request a consultation, or complete our appointment quiz, which may include your name, email address, phone number, and details about the dental services you're interested in. We do not ask for or store health/insurance information through these forms beyond what you choose to type into a free-text field.
